Does it mean my NAND is defect?

Yes, that's pretty much what this means. The driver that is attempting to initialize the NAND is failing on the first step -- it does not detect the NAND chip at all. This can happen if you've dropped the phone really hard and one of the solder joints has popped, but it can also happen if you use older, unrecommended tools to do a flash or jailbreak.

What happened to the phone prior to this error message? That will help determine whether it's something that may be able to be fixed by you or whether the best bet is to just buy a replacement logic board.
