You have likely updated your baseband to 5.14.02, this can't be unlocked by ultrasn0w.
Did you use ultrasn0w or blacksn0w to unlock previously?
Download TinyUmbrella from The Firmware Umbrella - TinyUmbrella - run it. Connect your iPhoen and reply with the information that TinyUmbrella shows (iOS version, baseband etc)Please read the stickies & search forum before posting!
