    Though, I still need further assistance, and I appreciate your efforts so far.

    Currently, my iPhone does not display the caller id while in call, but the names are shown OK in the recent tap.
    If I install your CallerID format fix FW 3.0 (I am using iOS 6.1.2, Firmware 3.4.02) the iPhone phone application crashes once I open it, and any caller keeps ringing me while I don't get notified. Moreover, the iPhone restarts unexpectedly and enters the safe mode!!

    Is there a solution for that??

    I am not surprised , "CallerID-Format Fix for FW 3.x" is valid for iOS 3.x and 4.x , uninstall it.
    "CallerID-Format Fix iOS 5.x" is valid for iOS 5.x and 6.x , but manual substitution is equivalent ,
    the App substitute two files but most countries (including Yemen) need only one , Default.phoneformat
    About caller-id not displayed during the call but OK in recent, I have no idea, sorry.

    i found how to fix caller id not showing name for Yemen, i tried this fix and it is working perfectly,

    using iTools 2012 or iFile open
    system/Library/Framworks/UIKit.framwork/PhoneFormats/UIMobileCountryCodes.plist
    edit with text
    1- go to this line <key>421</key> , and change this number 421 to 101 and DO NOT TOUCH THE SECOND LINE FOR THE COUNTRY

    2 - Then from line of number 310 To 316 ONLY change the country code ( us ) to ( ye ) .
    Finally save the file and exit , then restart the phone and you will find the problem solved,

    421 is the MCC (Mobile Country Code) assigned from ITU-T to Yemen,
    310 to 316 are assigned to US .

    I suppose you have an US-Locked iPhone with a SIM-Adapter , probably
    the adapter convert any SIM to MCC 101 (sometimes 001) or 310/316 .

    Changing us to ye in UIMobileCountryCodes.plist , at power-up ,
    country name (Yemen) is written in com.apple.AppSupport.plist and
    after used to interact with Default.phoneformat , so Caller ID works .

    I got iphone 4s CDMA locked to verizon , and now it is working with ( Yemen Mobile Company )
    in YEMEN , i was facing a problem with Caller name not showing when recieving a call infact the caller number
    and name is saved in my contacts, im not using any SIM-Adapter, my iphone is programmed with the company
    here in Yemen, i did the fix i wrote to you and my problem been solved and never come again.

    Thank you all, I managed to fix my iPhone by installing "CallerID-Format Fix iOS 5.x" with the new "Default.phoneformat" Frank8 posted here, and the "UIMobileCountryCodes.plist" modifications talaldn suggested. Thank you very much again!

    Talaldn, now I can't send messages the YemenMobile network keeps rejecting my SMS messages. Could you please upload your "Default.phoneformat" and "UIMobileCountryCodes.plist" files?

    open the file with Text Viewer then do the following :
    GO to the country code <key>421</key>
