Oh, sorry, my fault.
I have got no ideas left. I have about 40-50 bundle downloads per day no complains so far. I really dont know what went wrong on your iPhone.
If you had OpenSSH on the iPhone you could take a look into
/System/Library/Carrier Bundles
and check if it's in there, but from all you told me it has to be...
You should see a bundle named something like ...._custom.bundle in there plus the symlink (looks like a folder and has the MCC/MNC as foldername).

that's fine thanks for your prompt response. i will leave it for now until maybe more people report on this.. funny though as i only wanted to get rid of this stupid "feature" and can't even do that

i reinstalled the firmware this time with Cydia and the bundle has worked. I suspect Pwnage Tool won't install the bundle unless Cydia is selected for IPSW.

Ah, I see. To make the bundle working, BSD subsystem is needed and this is part of Cydia. T create a symlink btw you need the command ln ...
the bundle is working and will continue to work, even without Cydia.

my problem with Cydia is I experience slow down when it is installed. It was with previous 2.x version of firware and happens now. Without it my iphone responds very quick to any input.
EDIT: is there any way for me to log into the phone via USB? I would edit the carrier file this way.

1. On firmware 2.1, the MCC/MNC have been moved again, they can be found (after dialing *3001#12345#*) under "MM Info" -> "Serving PLMN" (they are labeled, like on 2.0, "Mobile Country Code" and "Mobile Network Code".
2. I've generated a custom carrier bundle on your site and it does not seem to work on my 2.1 3G. It *is* recognized correctly because the carrier logo has changed, but the "call forwarding - dismiss" popups are still there, also under Settings/Phone there is none of the shortcuts to my account url, voicemail, assistance etc (yes I've filled them in the online form and yes they show up in the carrier.plist within the bundle).
BTW I have a factory unlocked iPhone 3G v2.1 QuickPwn'd and used the manual SSH method to upload the carrier bundle to the phone.

This is really strange. The setting for the custom logo and "Call forwarding" is both in carrier.plist inside the bundle. So if one is executed, the other should be aswell.
Are you on one of Apple's partner carriers but without an iPhone plan?

Nope, I'm on a spanish "virtual" operator which isn't an Apple's official partner carrier. I've also downloaded from the iPhone the official bundle from the spanish official carrier and compared it to the custom generated one and the structure in the .plist files is pretty much identical, so I have no clue where the problem might be. 
It's weird that the custom logo is recognized though...

Also, the APN has been reset to blank even if in the custom bundle it was filled with the correct values.

Did you download the custom bundle recently? If you are on firmware 2.1, yoou know there is a new format. If you downloaded it a week ago or so it probably wont work.
This is kind of my last idea. Besides one more. Did ypu copy the bundle directly after downloadidg or did you open it before?
