The country is indicated by the model number of the phone. It's generally not possible to determine the specific network without trying different SIM cards.

The easiest way to determine what provider an iPhone is locked to is to take the IMEI and to call an Apple Store (not Apple Support, they will not work with you!). Just tell them you're looking at purchasing the device secondhand (or just did purchase, whatever) and that you need to determine the carrier the device is locked to. It's easier to get this kind of support in-person after making an appointment, but there are reports that getting the right 'Genius' on the phone will work as well. I can appreciate it's not the easiest method, but it's really the only way to determine 100% which network it is locked to, without testing a handful of SIM cards.
