The Three after the "PN" denotes the unlock counter has been exceeded.
The patched nor that the software (and hardware) unlock used only circumvents the requirement for a valid NCK (e.g. "00000000") !!!
It does not circumvent the Facility Lock/Unlock process which is why you still have to do the AT+CLCK command. If this has been entered wrong through typos, cutting & pasting errors etc, it does not take much to enter this command in wrong a few times!
As I have already said, this guy has re-flashed the FLS and the EEP files with virgin files and the lock counter has stayed intact so further circumvention is required (against the Lock Counter) for a perfect solution!

I believe at this point we need to find a way so that,the phone can patch every file that it has from the firmware, such that the counter is reset. i was going through firmware update section on wiki and as is written its not that difficult to customize the update process, i dont have a mac and things become a little difficult here, i wanted to try to customize update so that we can patch all the files ,hence regenerating no of attempts. I think this just probably slipped down the dev team's eye.
also can someone wit a mac try this?

Here's my results:
AT+XSIMSTATE=1:

The 3 indicates that it has reached the unlock attempt limit.
I've tried to unlock my phone many, many times(atleast 10-15 times). I'm not a computer rookie either. I know how to work around command lines. I've flashed and reflashed the baseband. Yet, I am still unable to get it unlocked.
However, I must admit, during the minicom procedures, I've made mistakes entering the AT+CLCK commands. It is extremely easy to make a mistake under minicom because there is no way to erase your mistakes (for some reason, the delete key doesn't work under minicom). This might have forced it to hardlock. Even with the new GUI, it still refused to unlock.
An unlocked phone should give a 4 instead of a 3.

I don't have this problem myself, I am just helping those that do have the problem identify what the problem is to save the going mad from going round and round in circles :-)
The people I have spoken to so far have been tearing their hair out because they do know what they are doing and have generally unlocked previous phones without issue, so hence they try and try and try again and can't understand why the same process that worked for them previously suddenly doesn't work. The Lock Counter is the reason!

thats preciously what happened with me,i dont know when exactly i attmepted that happened so that the counter reached the count,and also after banging my head for 3days ,i exchanged my phone from the retailer ,did the any sim process and unlocked in first attempt,i tried the bbupdater fls eep files and all several time b4 i decided to exchange the phone, but i still wish to find how i could have unlocked that phone,anyway i carry the same reason to be bein here

iUnlock_Reloaded does the AT+CLCK command on your behalf, so minicom issues won't be a problem going forward.

Quite frankly I don't trust any of these gui programs.
I know they had timing issues on the first versions, who knows what could be happening behind a pretty picture.
I am confident in my ability to enter the CLCK command myself, so I use the original iUnlock created by the Dev team but the revision with the higher baud rate.
It takes about 2 minutes to do it manually and you have the piece of mind that you can see each step going through successfully (e.g. the Erase, nor flash, secpack etc).
Its never failed me and because I setup my phones properly by putting anglocised files on and the relevant carriers personalisation stuff as well, using a GUI has no time saving benefit what-so-ever. I still want to SSH and SFTP to the phone.

Pure theory....Lets say, your iphone is unlocked. Could there be a way to get that counter on 3, so that even Apple no longer can relock?
