Restore the iPhone with iTunes.
How was it activated?

Try this then try a different unlock method: (For Windows only)
-Unplug iPhone
-Reboot Computer
-Open up iTunes
-While holding 'Home' on iPhone plug it into the USB cable
-Continue to hold 'Home' until iTunes shows your iPhone.
-Then click restore in iTunes
