I have the EXACT same symptoms and cleaning the SIM card contacts and SIM slot didn't fix the problem for me. I do not think that it is a heating issue since my phone is not warm to the touch.
But just to reinforce the symptoms, I do notice that the iPhone will turn itself on a couple times, each time, switching between "ringer" and "ringer (headphone) mode.
I have tried everything:
1. put headphones in, slowing pulling it out
2. restore iPhone
3. Put in headphones, open iPod app, stop music, close app, and pull headphones out.
4. Now the fix mentioned above.
None have helped so far. =( Any insight would be appreciated!

That was exactly what happened with my gf's white iPhone 3G. She left phone get down onto water and phone was stucked on headphones mode. After a couple hours, it was somewhat crazy but working. It would get on and off headphones mode at will.
Luckly it was under warranty and carrier replaced it with no complains. Best part is, she came home with a white 3GS

yeah it seems like the fixes for this problem varies soo widely!
People manage to get it fixed through cleaning it with alcohol, others just by unplugging and plugging in quickly.
There has been no consistant method for this problem, the only consistent one is to replace the flex cable. But thats not really a solution to some people i guess

Happened to me
Happened to my wife's iPhone 3G - 8G last night.
I was in a rush and tried to fix it by resetting, plugging and unplugging many times, power on / off. Cleaning with alcohol. Poking the jack with a paper clip.
Nothing helps!!!
Then a read a post in apple discussion by "sucking through the headphone jack really hard" and it WORKS !!!!!
(remember you have to suck really hard. It's definitely a hardware problem that some sensor wire got stuck in the jack or the motherboard. By sucking air out, you dislodge the connection. I imagine blowing in would make it harder to fix later on)

hello you might want to try this i got an ipod touch 4 and ringer , volume stuck on screen try rebooting unit and few other tricks, my unit came in contact with water a month ago but been working fine till this problem ,i solved this issue with a blow gun and a house compressor at least 80 p.s.i. turn unit off and point gun to ear plugs hole make sure you get a real good seal in there ,
that got my unit unstuck and working really good again couple of weeks no problems good luck .
