Looks good but please lets stay at the topic. If you like to start a thread on how to create cool looking carrier logos, would be great. Other users would love to check this out.

Can you add ability to change TEXT name of carrier by the carrier bundle?
My problem is:

on iPhone 2G and any firmware version my operator name is - TELE2
on iPhone 3G and firmware 3.0+ name is - EE TELE2 and this long name scrolling!

Thats what the last field is for: entering the text you want. Check all the values you entered in the form, something is probably wrong. Please read the complete how-to

lets not start a discussion on that topic here, it's old story. If not on a legit contract, in most cases you'll see scrolling text. it is generated by the provider/simcard. You cant do anything about it. Only way around: a custom carrier logo (graphic)

there is a page tha have a list of all the carriers in the world and let you download it to your iphone or email it to the acount that you have in the phone, and then install it like a profile or something, I dont remember the name of the page, can somebody give me some advise on this?

Unfortunately yes. With firmware 3.1 and iTunes 9 you still can inject ipcc files with custom carrier bundles. But even on my factory unlocked iPhone with a carrier allowing tethering, using the iPhone as a 3G modem is broken. I am working on this.
