I successfully installed and unlocked my phone using iphonesimfree but restored it and installed SimFree (1.5) for the second time.

Running iphonesimfree again, I get a dialog box that says "Already unlocked!, Ok"

Under the Settings menu, I no longer have the Carrier item. How do I get this back?

I believe this is a common scenerio since the phone may need to be restored from time to time.

Unlock will not be removed with a Restore. Take your sim out and put it back in. Hard boot the phone.

Also try to restart your phone after activation.
