Try Setings - General - Reset - Reset All

Plug your earsets in and out a couple of times

I've been inside there and the jack socket has a tiny brass connector which switches open and closed as the headphone jakc goes in and out, of course being mechanical, it is subject to failure, weak return springiness (neologism) or particals of dirt... not poor design as there is hardly an alternative unless they used sensors and we know how prone to faults sensors are if they are firmware based!!!
