It's not the md5 hash collision that I'm worried about. But when a pair of certs become available, how does the server ensure there will be no two (or more) clients downloading it at the same time? Due to high demand, there is a high probability that one client connects while another client download is in progress.

Which means the cert may not be deleted quickly enough. Unless the server makes sure to prevent concurrent downloads for the same cert. This is what I'm trying to establish here: are the certs really unique, or almost unique?

I'm checking with anethema on the server side, I will let you know to see.
As far as the certs being generated, they are 100% unique.

There are some cydia programs that can screw up push notifications.
SBSettings Push Toggle and AutoSilent both are known to cause this problem.
The fix is documented here: Twitlonger: @pushfix might have found the solution to the 'plug into iTunes'. It has worked for 3 people so far.

Duplicate download were very unlikely before, but since you've raised the question, PHP has been completely re-written and therefore it's now impossible for it to happen

The fix is simple. Problem will come back until you either stop using autosilent or if possible just disable the feature that silences push notifications.
