One thing to note also is that when it says call failure it says it twice I also noticed line 154 effects the MSG displayed when you drop a call.
If any one figures out how to make it show the caller's nsme id like to know as well, i'm going to keep plugging away to see if I can find it, might have to take a hex editor to the springboard.

Well I kind of appreciate the "Call Failed" message since the voicemail SMS notifications don't come through on the iPhone, it's nice to have a different message when someone has let the phone completely ring-out, as compared to someone just ringing for a couple of rings which just says "missed call" and has no chance of a voicemail being left. At least then I only need to dial my voicemail when i see "call failed"

i uploaded the files i modified as masimunes mentioned -

    SpringBoard.strings:
    http://www.send_space.com/file/f1a177

    USSD.strings:
    http://www.send_space.com/file/tkliqf
    (remove underscores)

back up your old files just to be safe and upload these files in the /System/Library/CoreServices/SpringBoard.app/English.lproj/ directory. Be sure to power off the phone afterwards and then this will change the both texts from Call Failure to Missed Call

This is bascially a workaround since both text fields are saying Missed Call, one of them should actually read the missed call's # or Name - but no one seems to know what to input (key/string) in order to retrieve the last missed call.

Still no fix for this? Changing those files also displays the missed call text when you have no service and attempt to make a call. So far this is the only problem with the unlock. Are all t-mobile users experiencing this?

paintball, this isn't something that has to do with the unlock apple chose to use this generic call failure as opposed to creating a seperate routine for missed calls.

thats not correct - my brother has an at&t activated and locked iphone and when he gets a missed call that goes to voicemail it appears exactly the same way as when it does not go to voicemail (display's the name of the missed call along with the text 'missed call' on the same line).
