In one earlier version, a setting "Disable device until next sync with iTunes" was included. I guess it's a certain brand of humor. It could apparently be undone with a sync to iTunes.
If that isn't your problem, perhaps iPhoneBrowser can help. Try to delete /Library/MobileSubstrate/DynamicLibraries/SupremePreferences.dylib. It may be responsible for the problem, because it attempts to change the lockscreen, apparently.
You think you install a change to your preference menu, but in fact you install a lot more. There's not just this, you also install WebViewController: another MobileSubstrate plugin, to put some weblinks and a Donate button in your Settings menu.
Anyway, I have already complained about iMario's design decisions
elsewhere (though including someone else's JavaScript bookmarklet "find in page" and naming it "SP 3.0: Find in Page" is a new low, I guess).
