was there a question of some sort? But ya, this is first for me.
This is a crash protection provided by MobileSubstrate framework, it is installed as a dependency with some packages like WinterBoard.
same here but look underneath the normal phone app button? there is another .app that you cant see... (also the icons are of alignment), it happened to me after modifiying the mobileinstaller... a restart solves the problem...

it's for protecting you against winterboard failz because it uses code injection for the plugins and if someone is a bad coder it could screw up springboard so then the backend that allows the code injection to be used will notice the failz and report it as well as put the device in safe mode so that you can disable the offending plugin.
your dock icons beeing like that is because 5 icon dock gets disabled in "Safe Mode" :) same thing happend to me when it went into safe modeQuote:
