Hi,
search for that in the forum. There are 3-10 threads, a bit older, which will deal with this. It basically happens, when you use carrier bundle that has this feature activated - which is usually the default one (unknown carrier).
The user volkspost has provided links to create your own carrier bundle, and there you can deactivate this message. Other solution would be to look for the Andre Sendowksi source and install his patch (which is basically changing the respective line in the carrier bundle package).
Cheers

Hmm, maybe a better topic? I thought this was about a solution to remove the popup, not a question.

This popup is removed by iBlacklist software for free. You donīt need to register for this to work.
Just perform these steps:
* Install iBlacklist version 1.9.3 from iclarified
* Reboot (itīs required)
* Access the app and put the Enable switch to ON
* Access the Extras view and enable only the Call Forward Privacy to ON
After exiting the app, you will face a normal respring. After this, no more Call Forwarding popup and/or Outgoing Call Forwarded popup will be present on your iPhone.
And, it wonīt filter any call, since your blacklist is empty. It will filter blocked and unknown ID calls, but you can permit them by going to Call Settings and switching the Restrict Number option to OFF. Now you will receive all calls and smsīs normally, and will just use the popup remover for the forwarding annoying popups.

For a cleaner (and slightly more technical) way to do this, see http://hackint0sh.org/forum/showthread.php?t=11641
In principle it's no more than a simple setting in a "plist" (Apple property list) file, so you shouldn't need to install applications like iBlacklist to accomplish this simple improvement.
Let us know if this poses any problems.

Installing Cydia programs on a phone that has no internet connection: read this.
Editing binary .plist, .strings, .nib and .xib files:
* on your computer: Windows tool / conversion website.
* on your iPhone: convert those files in a terminal with plutil (installed with Erica Utilities) or edit them with iFile (Cydia links).
