3 - Restart iPhone

6 - Restart iPhone

7 - Open Messaging app and look at the camera icon

NOTE: The camera icon only appeared after restarting the phone after I entered APN, MMSC, and MMS Proxy.

Yep, works like a charm. Both MMS and mobile internet working with different APN. I first installed a custom carrier IPCC file with iTunes and then installed activatemms2G. Reboot and worked from the first time.
