Default you can

I am on Pay Monthly. If you are on pay as you go you can have it unlocked after 1 year for £15.
If you are on pay monthly then you can have it unlocked at any time for free but you agree to see out the rest of your contract!!

They should update the hardware identifiers in their databases to reflect your refurbished iPhone when they swap out the iPhone under warranty. You should have no problems getting that iPhone unlocked.

I guess you are having a systematic problem with your device. Try to contact with the network operator and apple service center.
