Sorry to inform you, but....
This is African, it is typical Southern Regional workmanship! Judging by the huts in the background and the dress code, I would say it is in the Lesotho/KwaZulu Natal region in South Africa! Yes that is a Automotive Battery(12-19kg) and it is a Lead ACID cell Battery! They are commonly used as power in rural areas and are carried everywhere daily to have charged in order to provide electricity in the night(and of course power for a radio)!The balancing on the head is a skill learned by most African People and is not unique to Southern Africa but is used for carrying most things! Ladies carry 25l drums on water like that with problems except physical of course as this puts a massive strain on their necks and causes severe disabilities in older ages! There are many charitable drives to supply them with wheelbarrow kind of watercarts, but I have seen that only assists them so the can carry a 20kg bag of flour AND push the watercart at the same time!!
